Citigroup made the transfer in April, but it took 90 minutes before the mistake was noticed and reversed. The issue, which was later reported to the Federal Reserve, was quickly corrected with no impact on the bank or its client.

The incident, categorized as a "near miss," is part of a series of similar errors Citigroup has experienced. The bank stated that while the transaction was flagged and reversed due to its controls, the event highlights the need to continue automating systems and eliminating manual processes. Last year alone, Citigroup recorded 10 near misses involving sums of over $1 billion.

He blames unsafe conditions for the accident, telling Business Insider that he walked into an area with no warnings, right beneath a tower being disassembled. When a section of the tower collapsed, it crushed his shoulder and caused significant damage to his lungs.

Sam, who spent eight days in the hospital, is now seeking compensation. Both he and a supervisor claim there were no clear markings to indicate construction overhead, though another source says there was a marked path. Sam believes the whole incident could’ve been avoided.
